A practical checklist

  1. Choose Repair with the actual work date under the affected machine.
  2. Describe the symptom and completed action in factual terms.
  3. Record the post-repair check and any unresolved issue without claiming more than was tested.
Illustrative recordA worked example

Drill D1 · Repair · September 11. “Replaced damaged guard fastener after loose guard found. Guard secure on stationary check; field operation not tested today.” The limits of the check are explicit.

What to avoid

Do not write “fully fixed” if the machine has not been checked in the relevant conditions. A maintenance note should preserve uncertainty, not hide it.
